FOR IMMEDIATE RELEASE -November 20, 2012 - Carolina Prime Pet Inc., a manufacturer and distributor of dog treats, is voluntarily recalling the following product:


Priority Total Pet Care All Natural Bullstrips in a 5-count package

UPC: 0-21130-42080-3

Lot Code: 20082712 S 3195 and Lot Code: 20090312 S3195


The UPC can be located on the back label of each package. The lot Code is located on a sticker on the back of each bag of the 5 count product.


This lot of product has yielded a positive test for Salmonella when tested by the Colorado Dept. of Agriculture. Carolina Prime Pet is not aware of any reported cases of illness related to this product.


Salmonella can affect animals eating the products and there is risk to humans from handling contaminated pet products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products.


Healthy people infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. Rarely, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.


Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans. If your pet has consumed the recalled product and has these symptoms, please contact your veterinarian.


Priority Total Pet Care All Natural Bullstrips are sold in Safeway stores in Arizona, California, Colorado, Delaware, Hawaii, Maryland, Nebraska, Nevada, New Mexico, South Dakota, Virginia, Washington DC and Wyoming as well as Vons, Pavilions and Pak 'N Save stores in California; Randalls and Tom Thumb stores in Texas; Genuardi's stores in Pennsylvania and New Jersey, and Dominick’s stores in Illinois. This product was distributed from about the first of September until now.


Customers who purchased the recalled dog treats should discontinue use immediately, and return items to the purchase location for replacement, refund, or discard.

Carrot ChipsBolthouse Farms Voluntarily Recalls Limited Quantity of 16-ounce Carrot Chips

BAKERSFIELD, Calif., November 1, 2012 -- Bolthouse Farms is voluntarily recalling a limited quantity of Bolthouse Farms® 16-ounce Carrot Chips following a routine sampling event, conducted on October 22, 2012, by a North Carolina health official. The company was notified on October 31, 2012 that the inspection detected the possibility of Salmonella in the single16-ounce bag that was tested. Bolthouse Farms has not received any reports of consumer illness or other consumer complaints related to this product.

Out of an abundance of caution, Bolthouse Farms is retrieving the Carrot Chips from the marketplace.

No other Bolthouse Farms products are affected by this recall.

Approximately 5,600 cases of the 16-ounce bags shipped to retail customers in the United States and Canada are being recalled. The affected product is labeled as Bolthouse Farms Carrot Chips, Safeway Farms Carrot Chips, or Farm Stand Carrot Chips and has the following information and codes on the front right corner of the bags:

Salmonella is an organism that can cause a foodborne illness. Symptoms of infection include diarrhea, fever and abdominal cramps 12 to 72 hours after infection. The illness usually lasts four to seven days, and most persons recover without treatment.

Safeway Expands Recall of Ground Beef Products Sold in Four States
PLEASANTON, Calif. — September 25, 2012 — Safeway announced that it has been informed by Interstate Meat Distributors that some of the ground beef products produced for Safeway included trimmings that were subject to XL Foods Inc.’s expanded recall. In cooperation with the USDA Food Safety Information Service’s expanded public health alert on raw boneless beef trim products imported from Canada by XL Foods Inc. that may be contaminated with E. coli O157:H7, Safeway is voluntarily expanding its September 22, 2012* recall of ground beef items to include additional dates and states.

Safeway stores in Idaho, Montana, Oregon and Washington (State) are included in this recall of all fresh ground beef sold from the self-service meat cooler and the full-service meat counter with Sell By dates of 08-28-12 to 09-23-12. This includes all items that use fresh ground beef as an ingredient, including but not limited to meatballs, sliders and gourmet burgers.

In addition, the following pre-packaged products distributed by Interstate Meat Distributors with the specified Sell By dates are also subject to the expanded XL Foods Inc. recall.

E. coli O157:H7 is a potentially deadly bacterium that can cause bloody diarrhea, dehydration and, in the most severe cases, kidney failure. The very young, seniors and persons with weak immune systems are the most susceptible to foodborne illness.

The USDA Food Safety Information Service (FSIS) advises all consumers to safely prepare their raw meat products, including fresh and frozen, and only consume ground beef that has been cooked to a temperature of 160 degrees Fahrenheit. The only way to confirm that ground beef is cooked to a temperature high enough to kill harmful bacteria is to use a food thermometer that measures internal temperature.

Safeway Recalls Ground Beef Products Sold in the Northwest
PLEASANTON, Calif. — September 22,2012 — In cooperation with the USDA Food Safety Information Service's public health alert on raw boneless beef trim products imported from Canada by XL Foods, Inc. that may be contaminated with E. coli O157:H7, Safeway is voluntarily recalling ground beef items sold in Oregon, Washington State and Montana. Safeway was informed by Interstate Meat Distributors that ground beef products produced for Safeway included trimmings that were subject to XL Foods Inc.’s expanded recall.

No illnesses have been reported to date. Safeway is issuing this recall out of an abundance of caution.

The recalled products have Sell By dates of 09-07-12 to 09-23-12. Safeway is asking its customers to check all ground beef in their refrigerators and freezers and discard or return recalled ground beef to the place of purchase. September 19, 2012 – GHSW, LLC of Houston, Texas is initiating a voluntary recall of a limited quantity of expired products that contain fresh-cut mangoes and are distributed to retail supermarkets due to the potential risk that the mangoes may contain Salmonella. Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

This recall is associated with Food Source’s recall of mangoes sourced from Agricola Daniella. The FDA has issued an Import Alert and advised consumers not to eat mangoes from Agricola Daniella.

The products listed in this recall were distributed through retail stores to the following states: Alabama, Arkansas, Louisiana, Mississippi & Texas. Products are packaged in clear plastic containers (cups, trays and clamshell type containers) and were distributed to retail distribution centers August 29-September 5, 2012. Products contain printed code dates on the top or bottom labels of the plastic package, ranging from 9/7/12 -9/15/12. This is associated with FoodSource’s (Edinburg, TX) recall of mangoes sourced from Agricola Daniella. The FDA has placed Agricola Daniella on the Import Alert Listing.

There have been no reported illnesses attributed to the items listed in this recall. Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. F&S Produce Co., Inc. of Rosenhayn, NJ is initiating a voluntary recall of a limited quantity of expired products that contain fresh-cut mangoes, with a code date that is on or before August 28, 2012, as a result of a FDA recall by Splendid Products because Daniella brand mangoes distributed by this firm may be contaminated with Salmonella. Products containing fresh-cut mango products that were affected by this recall were distributed to grocery and convenience store customers in the Northeast. Products were processed between 8/9/2012 to 8/19/2012, and utilized mangoes sourced from Mexico that are associated with this current outbreak. Products produced with Use-by or Sell-By dates of August 29 or later, have been processed using mangoes sourced from Brazil, and are not associated with this recall.

GH Foods CA initiates voluntary recall of products that contain yellow onions
SACRAMENTO – July 27, 2012 – GH Foods CA, LLC, is initiating a voluntary recall of various products containing yellow onions distributed to retail supermarkets. The products were manufactured using onions recalled by Gills Onions because they have the potential to be contaminated with Listeria monocytogenes. Listeria monocytogenes is an organism that may cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea. Listeria infection can cause miscarriages and stillbirths among pregnant women.

A list of the brands, products and states is provided at the end of this release. No other GH Foods CA products or code dates are affected by this recall. The company is cooperating with the FDA. Safeway Signature Café Spaghetti & Meatballs Part of a Voluntary Product Recall


As part of a Class I recall by Buona Vita, Inc. a Bridgeton, NJ establishment, of approximately 324,770 pounds of various frozen, ready-to-eat meat and poultry products that may be contaminated with Listeria monocytogenes (referenced as FSIS-RC-042-2012), Valley Fine Foods Company, Inc. is notifying consumers that Signature Café Spaghetti & Meatballs, contained meatballs supplied by Buona Vita.


Signature Café Spaghetti & Meatballs is sold exclusively at Safeway and Safeway-owned stores throughout the U.S., including Carrs, Dominick's, Genuardi's, Pak 'N Save, Pavilions, Randalls, Tom Thumb and Vons. The product, sold in the refrigerated deli area, is packaged in a microwaveable tray with a net weight of 30 oz. (850 g) and has the UPC Number 0-21130-06875-3. The affected packages were sold from May 26, 2012 through July 9, 2012 and have Use By dates of Jun 30 12, Jul 01 12, Jul 05 12, Jul 07 12, Jul 08 12, Jul 11 12, Jul 14 12, Jul 18 12, Jul 19 12, Jul 21 12, Jul 25 12, Jul 27 12, Jul 28 12, Aug 01 12, Aug 05 12, Aug 07 12, Aug 08 12 and Aug 11 12. The "Use Components By" date can be found on the side of the sleeve.


No other Signature Café items are affected by the recall. No illnesses have been reported.


The Buona Vita recall was issued after Listeria monocytogenes was discovered through microbiological testing by FSIS and the Ohio Department of Agriculture. Listeria monocytogenes is an organism which can cause serious and sometimes fatal infections in young children, frail, or elderly people and others with weakened immune systems. Although healthy persons may suffer only short-term symptoms such as high fever, severe headaches, stiffness, nausea, abdominal pain and diarrhea, Listeria monocytogenes can cause miscarriages and stillbirths among pregnant women.

August 10, 2012 – Missa Bay, LLC, a wholly owned subsidiary of Ready Pac Foods, Inc., of Swedesboro, New Jersey is voluntarily recalling a total of 293,488 cases and 296,224 individually distributed units of fruit, vegetable, and sandwich products containing apples, as listed below, with the Use‐by dates of July 8, 2012 through August 20, 2012 because they contain diced or sliced apples which may be contaminated with Listeria monocytogenes. Listeria monocytogenes is an organism that can cause serious or life‐threatening food borne illness in a person who eats a food item contaminated with it. Symptoms of infection may include fever, muscle aches, gastro intestinal symptoms such as nausea or diarrhea. The illness primarily impacts pregnant women and adults with weakened immune systems. Most healthy adults and children rarely become seriously ill.

Salinas, CA – Pacific International Marketing ("Pacific") is voluntarily recalling 19 cases of bulk Romaine Lettuce sold at Vons and Pavilions stores in California and Nevada due to potential Salmonella contamination. The bulk Romaine Lettuce was sold in bulk produce bins from July 2, 2012 through July 4, 2012. The lettuce heads are banded with a red twist tie marked "Safeway."


Consumers who may have purchased this product should discard it or return it to the place of purchase for a full refund. No Illnesses have been reported to date. This recall is being conducted out of an abundance of caution. The recall is the result of a positive Salmonella test taken at the field level. The source of contamination is unknown.


Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

For Immediate Release: June 7, 2012 – Windsor Foods is voluntarily recalling Safeway Select Five Cheese Lasagna (80 oz.) due to a potential labeling error. The code date on the retail carton reads as follows:
  • Best Before: S1388 APR 04 13 2922095A
  • Best Before: S1388 APR 04 13 2922095B
  • Best Before: S1388 APR 04 13 2922095C
  • Best Before: S1388 APR 04 13 2922095D

All affected product has the same date, with a distinguishing letter at the end of the code designating the portion of the day it was made (A, B, C, or D). It can be found on the endflap of the carton.

The packages may have been mislabeled and may actually contain Safeway Select Meat Lasagna, a product that contains the allergens of soy and egg, which are not declared on the Safeway Select Five Cheese Lasagna carton.

People who do not have a soy or egg allergy or sensitivity can consume the product without concern.

No other Safeway Select Lasagna products are affected. The product was sold at Safeway, Carrs, Dominicks, Genuardi’s, Pak ‘N Save, Pavilions, Tom Thumb, Randalls and Vons stores in the below-listed locations.

B&M, INC. Conducts Voluntary Recall of O Organics Organic Celery Seed Sold at Safeway Nationwide Due to Possible Salmonella Contamination
B&M, Inc. of Mount Vernon, Missouri is voluntarily recalling O Organics Organic Celery Seed sold in Safeway stores due to potential Salmonella contamination. The product is sold at all Safeway-owned stores, including Safeway, Carrs, Dominick’s, Genuardi’s, Pak ‘N Save, Pavilions, Randalls, Tom Thumb and Vons.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

O Organics Organic Celery Seed is packaged in a glass bottle with a net weight of 1.8oz (51g) and marked with a UPC 0-7989341124-8. The product was sold at stores from May 25, 2011, through December 16, 2011. Bottles with the following lot codes and "Best By" dates are subject to the recall:

B&M, Inc was notified by their supplier that a lot of Organic Celery Seed had potential Salmonella contamination. The recall is based on a positive result for salmonella that was found during routine testing.

Consumers in possession of this voluntarily recalled product may return it to the store at which it was purchased for a full refund.
